Grayslake Park District Questionnaire
The Grayslake Community Park District (GLPD) is launching a strategic planning initiative and is seeking input from community members to help guide future programs and services.
We invite all community members age 18 and older to complete a brief questionnaire to share ideas, suggestions, and feedback. The survey includes 14 questions and should take less than 10 minutes to complete. Participants must live in Grayslake, IL or Hainesville, IL.
Your input will play an important role in shaping the future direction and operations of our park district.
The questionnaire was developed in partnership with the University of St. Francis (Joliet, Illinois), and all responses will be analyzed by the university. Responses are completely anonymous and will not identify individual participants.

We will have paper copies available in person at our Recreation Center located at 240 Commerce Drive, Grayslake, IL. To honor your privacy, please place your completed questionnaire in the drop box located at the south entrance of our parking lot.
