Once you register for summer camp, please complete the following forms so we have all the information we need for your camper!

Automatic Payment Form
The automatic payment option is only available for Summer Fun 2021. Preschool Camps must be paid in full at the time of registration. If you’d like to take advantage of the automatic payment option, please submit the automatic payment form with your registration form. Unfortunately, the option isn’t currently available for online registration.
Emergency Forms should be returned to the Grayslake Park District prior to the start of camp. Forms can be submitted via email firstname.lastname@example.org, mail (Attn: Kendra Ring, 240 Commerce Drive, Grayslake, IL 60030), fax (847-223-6386) or dropped off at the Recreation Center.
